RGA / License Certification Form
A RGA / License
Certification form is required each time a Nuclear Gauge is returned to
a customer. North Carolina regulations require that the customer is licensed
to possess radioactive materials of the type, quantity and form assigned to
a specific gauge before returning the gauge to the customer. Complete this
form to expedite the return of the gauge.
